Step 2 of 6 — FCRA Disclosure
Background check disclosure
This is a standalone disclosure required by the Fair Credit Reporting Act (FCRA),
15 U.S.C. § 1681b(b)(2)(A). It contains only the disclosure — nothing else.
Disclosure Pursuant to the Fair Credit Reporting Act
BreakCheck may obtain a consumer report (background check) about you from Checkr, Inc.,
a consumer reporting agency. This report may include information about:
— Criminal history (federal and county records)
— SSN trace and identity verification
— Sex offender registry status
— Global watchlist and sanctions screening
This consumer report will be used solely to evaluate your eligibility for BreakCheck
Certified status — a voluntary certification program for card hobby sellers.
This disclosure is made in accordance with the Fair Credit Reporting Act, 15 U.S.C.
§ 1681b(b)(2).
Your rights under the FCRA:
You have the right to obtain a free copy of your consumer report from Checkr
and to dispute inaccurate information. A summary of your rights is available from
the Consumer Financial Protection Bureau:

a consumer report about you from Checkr, Inc.

report about me for the purpose of evaluating my eligibility for BreakCheck Certified status.
I understand that this authorization applies to my current application and to any
subsequent annual renewals or re-checks conducted during the term of my certification.
Ethics standards acknowledgment
BreakCheck Certified sellers are held to documented standards. Read each statement
carefully and indicate your understanding. This is a commitment, not a scored test.
Every box must be checked to proceed.
Background check
Your background check is conducted by Checkr, Inc., an accredited consumer reporting agency.
Checkr will send you a separate, secure link to submit your personal information directly.
Your data goes to Checkr — not to BreakCheck.

Results are provided to BreakCheck as a pass/no-pass determination only.
You have the right to dispute any information with Checkr directly.
BreakCheck does not receive or store your raw background check data.
Timeline: Most background checks complete within 1–3 business days.
You will receive an email from Checkr with a secure link to submit your information.
BreakCheck will be notified when your check is complete.
Payment
Payment covers the Checkr background check fee, ethics training, and your first year of
BreakCheck Certified status. You will not be charged until your background check clears.
